Emergency Form (All families must complete an emergency form and submit to the middle or upper school electronically. This form must be on file at school before a student partakes in athletics or Camp Jewell.)

Image Release Form (All families must complete an image release form for each of their daughters and submit electronically to n.roque@holychildrye.org.)

Health Forms

Health Certificate Appraisal Packet (Each student must have a health certificate appraisal packet on file at school. Each year, the packet must be completed by a physician and submitted via mail or fax. This packet includes a physical exam, immunization record, health history questionnaire and non-required dental form.)

Athletic Health History Form (In addition to the health certificate appraisal packet, every athlete must submit this form, completed by a physician, and sent to school via mail or fax.)

Athletic Health History Update Form (This form must be completed by athletes at the beginning of each season and submitted electronically.)

Medication Administration Form (If a student must take medication at school, this form must be completed and sent to the school nurse.)

Technology Forms

**Seventh grade students in September 2016 will be using iPads, not laptops, in class. Returning students may continue to use their iPads from sixth grade. New students entering seventh grade in September 2016 must bring an iPad to school.

All students in grades 8-12 must have a laptop. Please read the laptop letter below for information on ordering a laptop for school.
