As the school year comes to an end, it's time to schedule physical exams and update NYS immunization requirements for the 2018-2019 school year.
- IMPORTANT: All NEW students and students in grades 5, 7, 9, and 11 are required to complete this form (provided by the Harrison School District) and provide an updated immunization record. Please remember to bring it with you to your child's physical. It is due back to the nurse's office by AUGUST 1.
- Sixth graders require a Tdap vaccine.
- Seventh and Twelfth graders require a meningococcal vaccine.

Fall Pre-Season for Upper School students begins on August 16, 2018.
Your daughter will need these forms that are required by the Harrison Central School District:
A Physical Exam that is less than one year old. So, if your daughter's exam was before August 16, 2017, she will need a new exam from her doctor to participate in Pre-Season.
An Interval Health History Form for Athletics that needs to be printed out, filled out, and signed by parents before August 16, 2018 for Pre-Season participation.
All medications needs to be brought into school before Pre-Season begins. There should be one set for the Nurse's Office and one set for the Athletic's Office. Medications can be left with Ms. Hajjar (Upper School), Ms. Koehler (Middle School) or Ms. Felter (Head's Office).
We would like Middle School athletes to submit all forms by August 16, but please no later than September 4. Students in grades 5 and 6 should write in the appropriate grade on the health history form.
All students are required to purchase to textbooks. These items are made available through an online bookstore that offers textbooks for direct purchase to families. New, used, and electronic books are offered for sale or rent. Students are encouraged to keep textbooks in good condition for potential resale at the end of the school year.
The Holy Child Online Bookstore through eFollet will go live on August 1. Be on the lookout for more information in late July.

Students who attend non-public schools may be eligible for the Book Loan Program. This program allows students to loan textbooks from their individual school districts. Please be aware that each district has different guidelines as to how much they allot for each student. Each district also has strict deadlines for submitted applications; some applications were due as early as June 1. Parents must contact their school district directly for information and requirements.

The Fall 2018 Sport Sign Up form is to be completed by the student-athlete. Please click here to submit your response, or update your prior submission. This form is limited to Holy Child email accounts, so parents, you will not be able to complete on your daughters behalf.
We will hold Open Gym & Field hours for any student (not necessarily athlete) that would like to utilize your facilities this summer! If you would like to use the facilities outside of these hours, please contact Ms. Conway regarding the schedule.
