As the school year comes to an end, it's time to schedule physical exams and update NYS immunization requirements for the 2018-2019 school year.
- IMPORTANT: All NEW students and students in grades 5, 7, 9, and 11 are required to complete this form (provided by the Harrison School District) and provide an updated immunization record. Please remember to bring it with you to your child's physical. It is requested back to the nurse's office by AUGUST 1, but please no later than August 15. Athletes especially should get their forms in as soon as possible to expidite clearance to play.
- Sixth graders require a Tdap vaccine.
- Seventh and Twelfth graders require a meningococcal vaccine.
If you have any questions, please email nurse Gina Romeo or call ext. 217.
Fall Pre-Season for Upper School students begins on August 15, 2018.
Your daughter will need these forms that are required by the Harrison Central School District:
A Physical Exam that is less than one year old. So, if your daughter's exam was before August 15, 2017, she will need a new exam from her doctor to participate in Pre-Season.
An Interval Health History Form for Athletics that needs to be printed out, filled out, and signed by parents before August 15, 2018 for Pre-Season participation.
All medications needs to be brought into school before Pre-Season begins. There should be one set for the Nurse's Office and one set for the Athletic's Office. Medications can be left with Ms. Hajjar (Upper School), Ms. Koehler (Middle School) or Ms. Felter (Head's Office).
We would like Middle School athletes to submit all forms by August 15, but please no later than September 4. Students in grades 5 and 6 should write in the appropriate grade on the health history form.
All families that purchased laptops outside of Holy Child need to bring them to Edward Yan to configure to our network and download the appropriate software. Please set up an appointment with him at e.yan@holychildrye or x233.
New 6th grade, 7th grade, and all 5th grade students need to have their iPads configured prior to the start of school. Please drop off the iPad to Michelle Sherry and include the Apple ID, password and passcode on the iPad.
Additionally, if parents set the Approve to Buy option on the student's Apple ID, that temporarily needs to be disabled (through iTunes).
Online book ordering will be available on August 1 through Follett Virtual Campus at www.holychildrye.bkstr.com. The link is also accessible through our parent portal. Please contact Eileen DiIorio at extension 237 or via email email@example.com if you have any questions. You will have the option to purchase either an eBook or a print copy, unless otherwise noted. When choosing a format, consider your daughter's learning style and preferences. Please note that only eBooks ordered through Follett will interface with our iPads and laptops. You cannot purchase eBooks from another vendor. The homework planner is mandatory for all students. Follett will provide half-price shipping for the first three weeks after the virtual site goes up.
Students who attend non-public schools may be eligible for the Book Loan Program. This program allows students to loan textbooks from their individual school districts. Please be aware that each district has different guidelines as to how much they allot for each student. Each district also has strict deadlines for submitted applications; some applications were due as early as June 1. Parents must contact their school district directly for information and requirements.
